All you need for an appointment

In preparation for your visit to our surgery, we would ask you to print out and complete the registration form. Due to data protection legislation, completing and sending the questionnaire online from our website is not permitted. You may, however, download this page as a PDF file. To ensure optimal care for our patients, we would ask you to provide us with a list of your medications and preliminary clinical findings. Please also read the Instructions on the examinations and how you should prepare for them.
